This questionnaire aims to honor the memory of loved ones killed by police officers in Ohio and provide a platform for their loved ones to share their stories and experiences before the tragedy.

Ohio Families (OFUAPB and OFUPAC) would like to do spotlight posts in our newsletter and on our social media shining a light on the wonderful person your loved one was and the light that was taken when they were killed by police. You can share as little or as much as you are comfortable sharing.
